In accordance with USCAP Policy and ACCME guidelines, the faculty members for this Companion Society
Symposium were required to complete disclosure forms indicating whether or not they (or their spouse or
partner) have any significant financial or other relationship with a commercial company, entity, or
service (which would be discussed in this educational program) which might be perceived as influencing
the content or conclusions reached in this presentation, currently or in the previous twelve months.
Relevant disclosure information for speakers is posted on the website at the beginning of each Companion
Society handout. The Academy also requires that the speakers disclose any "off label" uses of
pharmaceutical products or medical devices prior to their presentation.
As previously, paper handouts are not distributed at the meeting. Handouts for Companion Society symposia are being placed on
this page as we complete them so they can be viewed or printed at your convenience before,
during, or after the meeting. We feel this meets the needs of those we serve in a way that makes sense
by using electronic tools to make the information available at their convenience on their own computer.
Handouts accessible online, anytime, anywhere - and they will remain available after the meeting!
